When completed, you can either fax us your completed forms or bring them with you to your first appointment.

What do I need to bring on my initial evaluation?

Your physical or occupational therapy prescription
Any applicable insurance information
Photo ID, such as a Drivers License
Medical history, including symptoms, tests completed by your physician, and a list of your medications
Minors must have a parent or guardian with them, unless documented authorization has previously been received.
Wear comfortable clothes
